- Salary Range Band 6 ($93,459 - $101,873) plus 11.5% Super
- Full time 38 hour week
- St Kilda town hall with Remote Working Agreements possible.
About the Organisation
The City of Port Phillip is an inner-city council with around 1000 employees who deliver more than 100 programs and services to our residents, businesses and visitors. These range from childcare and immunisations, to libraries, management of parks and open space, rubbish collection, road maintenance and more. About the Role
Mange the social connection service which is an important component of the new “Village Model” which is Councils commitment to supporting people to age well in the City of Port Phillip. The social connection service delivers support through the Commonwealth Home Support Program, building social connection for people living within the City of Port Phillip. The social connection program delivers a variety of programs and activities which support older people to be actively engaged within the local community.
Duties:
- Deliver a responsive and coordinated social connection program within the scope of the Commonwealth Home Support Program for clients 65 years and above.
- Ensure that the social connection program meets and exceeds the legislative, accreditation and quality care standards.
- Manage a team of staff in the effective delivery of social connection services and high quality client outcomes.
- Proactively identify opportunities to grow, develop and improve the service based on community needs and aspirations.
Skills and Experience
- Tertiary qualifications (degree) in social services, health sciences or relevant field.
- Previous experience in delivering CHSP funded services and strong understanding of the Aged Care Quality Standards.
- Demonstrated experience in leading, supporting and developing high performing teams.
- Relevant experience in the delivery of community connection services, including the development of service offerings which respond to changing community needs.
- Experience and understanding of working with diverse communities and working with older people.
