Outreach Maternal and Child Health Nurse 

  • Salary Range $116,346 - $130,200 (pro rata) plus MCH qualification allowance and 12% Super
  • Permanent Part Time Role (22.8 hours per week)
  • Located at MCH Nurse centres located across City of Port Phillip

About the Role

The Maternal and Child Health service has an exciting opportunity for a Maternal and Child Health Nurse to outreach and engage families who may face barriers to accessing a MCH Centre to complete Key Ages and Stages in a childcare or home based setting.  Reporting to the Team Leader Maternal and Child Health you will provide child and family health focused services to our socially and culturally diverse community. Contributing to the implementation and facilitation of the State Government goals, objectives, performance measures and targets as prescribed in the current Maternal and Child Health Service Program Standards and the Maternal and Child Practice Guidelines 2019. We are looking for an enthusiastic and innovative team member with well-developed problem solving, decision making and counselling skills who can work effectively with families to support individual needs If you are a looking for a role where you can genuinely make a difference, and want to work with a friendly and supportive team

Duties:

  • Provide a Maternal and Child Health program following the ten Key Age and Stage Service Activity Framework. 
  • Monitor maternal and family health and wellbeing at home and childcare-based visits, and provide advice, information, support and referral as appropriate.
  • Assist with work in different locations within the municipality to meet the demands of the community as they arise.
  • Maintain accurate documentation and data on CDIS according to the Department of Health. Prepare reports as required by City of Port Phillip and the Department of Health.
  • Participate as an authorised professional in the Child and Family Violence Information Sharing Schemes. This involves making, receiving, and responding to requests, as well as proactively sharing and updating responders post sharing.
  • Monitor centre safety, cleaning and building maintenance requirements.

Skills and Experience 

  • Post Graduate Diploma in Child and Family Health Nursing OR Post Graduate Diploma of Nursing Science in Child Family and Community Nursing.
  • Current registration as both a Registered Nurse and a Registered Midwife (AHPRA registration).
  • Exceptional communication and interpersonal skills.
  • Strong organisational abilities and the capacity to manage competing priorities.
  • Knowledge and understanding of family partnerships, child development, and child behaviour.
